This document describes all of the ACSR related procedures including ordering materials, returning materials, administering exams, returning exams, querying the ACSR database, and appealing decisions related to ACSR. Please print this document and use it as a reference.
ACSR Order Form and Retake Information Sheet
As the Education Director of an IIABA state association that offers the ACSR program, you can order ACSR study guides and examinations. Here is the form you need to place your study guide and examination order. You also should use this form to order retake exams.

ACSR Certificate, Waiver and Credit Request Form
This is a multi purpose form used to submit student and grade information, ACSR Certificates of Completion and waiver requests.
AAI Segmented Exam Waivers Form
Students will use this form to request credit from the Institutes for AAI segments for which the Institutes does not have records, based on ACSR course completion. Although the states will not necessarily be involved with these requests, we have provided this form in case a student requests your assistance.
ACSR Exam Authorization Form
Because ACSR exams are secure, national exams, the exams will be shipped only to you, the Education Director of the state association. However, you can authorize another person to receive the ACSR exams and to keep them secure. Here is the form you need to provide such authorization.
Test Administrator's Affidavit
This form is used by test administrators to verify that exams were administered according to Institute standards and to note any problems encountered. A separate affidavit must be submitted with each set of exam booklets returned.
Test Administrator’s Guide
This guide contains all information pertinent to administering an ACSR examination. Rules within this guide, especially those regarding exam security, must be followed to maintain the integrity of the examination process.
ACSR Appeal Form
If an education director wishes to appeal a decision made by the Institutes, VU, or IIABA, he or she would use this form to describe the situation and ask that the ACSR Advisory Committee reconsider the decision.
List of Examinees Form
This form is used by the test administrator to track the number of exams given and returned to the Institutes. The back of the form is used to report general information about the examination and to report any problems encountered or suggestions for improvement in the process. A copy of the form is sent with each exam order.
Testing in Progress Sign
This is a “Quiet Please” sign that can be posted outside the examination room to alert others that a test is being administered.
Exam Information Card
This is for the test administrator to complete and post in front of the classroom if necessary.
